Transform will send you an invoice at the end of the month for the services received. All information will be provided to file a claim to your insurance company for reimbursement. Transform does NOT bill insurance directly. If you have any questions regarding billing your insurance Transform is happy to support you any way that we can.

Typical costs for 3 months of therapy

Recommendations will be based off your child’s evaluation and individual needs to meet the agreed upon goals. Family preference will be taken into consideration as able. Evaluation rate=$200 Treatment hourly rate=$140

  • Eval only $200 (Eval can be applied to any other package below)

  • Eval with Home Exercise Program (HEP) + 30 minutes follow up visit to check HEP =$275

  • 12 weeks 30 minute Physical Therapy Packages-must include eval

    • 1 x/week for 30 minutes ($840) + $200 Eval=$1040 total or $347/mo or $87/wk

    • 2x/week for 30 minutes ($1,680) + $200 Eval=$1,880 total or $627/mo or $157/wk

    • 3x/week for 30 minutes ($2,520) +$200 Eval=$2,720 total or $907/mo or $227/wk

  • 12 week 1 hour Physical Therapy Packages-must include eval

    • 1x/week for 1 hour ($1,680) +$200 Eval=$1,880 total or $627/mo or $157/wk

    • 2x/week for 1 hour ($3,360) +$200 Eval=$ 3,560 total or $1,187/mo or $297/wk

    • 3x/week for 1 hour ($5,040) +$200 Eval=$5,240 total or $1,747/mo or $437/wk

  • Intensives

    • 1 week for 5 consecutive days for 30 minute sessions ($350) +$200 Eval = $550

    • 1 week for 5 consecutive days for 1 hour sessions ($700) +$200 Eval= $900

Why is treatment for at least 12 weeks?

  • Research supports children’s motor learning occurs by two primary pathways; neuromotor reeducation and muscle strengthening. For motor learning, feedback needs are different in children then adults. For muscle strengthening, there has to be progressively increasing repetitions for a minimum amount of time which is around 8 -12 weeks. It takes 12 weeks long as I know that families are busy and it can be hard to carve out the time for motor learning and strengthening. I do not want to waste your time or money (which unfortunately could happen) if you only come once every other week or less and motor learning does not occur. If we decide to pick a once a week package most of the motor learning will occur with you in the home requiring consistent HEP follow through and me only progressing your child each week.

  • 12 weeks is an effective amount of time for your child to become comfortable with therapy expectations, make progress and reassess where we are and how far we have come.

  • A new eval is needed before each new package of treatment sessions to provide objective data to monitor progress.

Why is this my model of care?

  • No surprise bills or denied claims for either of us (this supports a much more sustainable health care system).

  • Holds both of us accountable for doing the work and meeting our goals in a controlled amount of time (therapy gets done quicker-no middle man).

  • It’s never ONLY what happens in the therapy session that creates the change in your child. We need to become a team; therapist, parent/caregiver and child.

  • Stops the common patterns of endless therapy, slowly reducing or stop coming over time and feelings that it didn’t work.

  • Goal is to learn a new motor skill and then use and develop it for awhile. This is why there are classes and groups and events to continue advance learning with peers. More episodes of care may be needed and could include different packages or intensives throughout the year.

  • Your child is worth it! You are worth it! I am worth it!

Payment Options

  • Payment is required for the first month (including Eval) at time of service and an invoice will be sent via email.

  • Insurance is not billed directly by Transform to keep costs down but a superbill (with all needed codes) is provided each month for you to send into your insurance for possible reimbursement. Ask questions as reimbursement is your child’s right! You just need to know how to send the correct information to get reimmbursed.

  • Payment options include credit card, Paypal and Venmo.

  • Vary depending of length of group, class or camp.

  • DDA funds may be available in the future (currently in application process) but need 1 month prior authorization to group or class.
