Transform Pediatric PT bills at the end of each month. You will receive a monthly invoice with all services rendered. This is called a “superbill” . It has all information needed to submit a claim to your insurance for reimbursement. You will need to be pay the full invoice directly to Transform within 1 week.

Transform Pediatric PT is open on Mondays, Wednesdays and Fridays with the highest frequency of treatment being 3x/wk. There are several weeks throughout the year where Transform will offer an intensive week opening up Monday-Friday for therapy. Many children and families have benefited from receiving daily therapy during these intensive weeks and have learned new skills like rolling, crawling, running and kicking a ball. Intensive practice is supported in the literature.
